Application form
We review all applications and will issue devices as soon as possible. Due to the current demand for our service, we aim to respond to applications within four weeks.
Please be aware that there is sometimes a waiting list for devices, even if an application has been approved.
Please note that we only accept applications from organisations such as charities, schools, local councils, statutory bodies or registered voluntary groups. Organisations must have attended one of our Referral Partner Onboarding sessions before we can issue a device.
There is now a £25+ VAT administrative fee for each device applied for, if your application is unsuccessful the admin fee will be refunded.
If you are an individual, please contact an organisation who knows you and ask them to apply to us on your behalf.
We will contact you by phone or by email if we have any queries. If the application is successful, we will email an offer letter, reference number and our terms and conditions.
Our Terms and Conditions need to be signed and returned to us before we can arrange a collection appointment. You can find our more about the Terms and Conditions by attending one of our Referral Partner Onboarding Sessions.
